Our Company Abode Care Partners Overview Under supervision of the CMO, you will have clinical and administrative responsibilities.
While providing day-to-day clinical care to frail institutional and non-institutional patients and residents, you will practice and role-model ethical and evidence-based decision-making to promote the vision and mission of the company to other practitioners and staff.
As a regional, administrative physician lead, you will provide medical director services in various settings, while training, mentoring and overseeing company nurse practitioners and setting accountability for key outcomes.
Part-Time and consulting options are also available for this role.
Responsibilities Assumes medical directorships for up to 5 skilled facilities and/or other senior living institutions.
(Details of medical director responsibilities will be provided as a separate document)Provides collaborating physician duties for up to 15 nurse practitioners and physician assistants (based on state regulatory parameters)Provides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioner and other interprofessional team membersParticipates in company frontline /Backup Call structures for timely response toto acute changes of condition to prevent avoidable hospital transfers, or other clinical needs.
No more than 5 days of frontline call per monthAssists company leaders in various programs to promote quality of medical services e.

, new and ongoing practitioner training, practitioner recruiting and hiring, performing chart reviews and audits, and to review quality outcomesKeeps a high focus on key practitioner and company level outcomes including but not limited to care path compliance and productivity, prevention of avoidable transfers and decrease in polypharmacy and costs of cateKeeps in regular communication with the VP of Business and Medical AffairsAttends at operations meetings, quarterly trainings, or other meetings as requested by the CMOAs requested by the CMO, participates in the review of, and discussion to improve financial outcomes of the designated region/ marketAs requested, participates in the review of attaining desired health outcomes for the designated regionPartakes, and as needed conduct, monthly meetings with providers to discuss medical topics and/or protocol and any items of concern encountered during the monthBased on state and regional regulatory frameworks, audits up to 10% of practitioner medical orders and/ or documentation for each provider on a monthly basis, providing a summary report to the Director of Quality Audits Qualifications Graduate from an accredited school of medicine and is a Doctor of MedicineBoard certification in Family Medicine, Internal Medicine or Geriatrics; qualification in Geriatrics preferredPracticing physician currently licensed under the provisions of the State Board of MedicineValid license to practice medicineTwo or more years of administrative leadership experience in healthcareClinical medical experience of seven years or more preferredStrong interpersonal, communication, and illustrating abilitiesExcellent computer and electronic record-keeping skillsExcellent organizational and management skillsExcellent decision-making, problem-solving, and cognitive abilities About our Line of Business Abode Care Partners is a leading provider of integrated medical services caring for individuals from post hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.
